Integrated Health Care Project
This project is a collaborative partnership between the UIC College of Nursing and Thresholds, a Psychiatric Rehabilitation Center. The purpose of the project is to deliver primary health care services to a population of chronically mentally ill adults that attend Thresholds programs. Studies have shown that persons with chronic mental illness have higher rates of physical health problems then the general population and have difficulty accessing health care services. This project proposes to address this problem by developing a model of care that brings health care services to the community setting where persons with chronic mental illness are comfortable in and have access to. Two nurse managed centers are located within the context of Thresholds buildings. A team of Family Nurse Practitioners and Psychiatric Clinical Nurse Specialists, who are on faculty at the College of Nursing, provide comprehensive primary health care services that are tailored to the specific needs of this vulnerable population. The centers also serve as excellant clinical experiences for both the undergraduate and graduate nursing students. The project began in March 1998 and the advanced practice nurses have seen over 900 adults and children for a total of over 10,000 visits in the first 4 years.
